Fresenius Kidney Care is one of the largest providers of dialysis services in the world, supporting patients managing chronic kidney disease.

PatientHub is Fresenius's digital platform for helping dialysis patients track their treatment data and stay connected with their care teams. The existing therapy flowsheet relied on paper-based documentation, which was hard to maintain and easy to lose track of.

As the visual designer on the project, I redesigned the flowsheet into a more intuitive, visually appealing digital experience. I created three clickable prototype variations, ran usability testing sessions with real dialysis patients to observe how they moved through each design, and worked with strategists to iterate on changes based on the feedback we gathered.
